Register with the surgery

We provide NHS GP services for patients living in the following postcode areas:

  • S64 – 0, 5, 8, 9
  • DN12 – 2, 4

We use an online service called Register with a GP surgery that makes it easy to register with this us.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

If you prefer, you can still visit our reception desk and ask for a paper registration form.

Once registered, you will be invited to attend a new patient health check. This helps us understand your health needs and gives you a chance to learn more about the practice.

What happens next?

  • We set you up on our system (EMIS).
  • We may book you an initial appointment (e.g. medication review, pill check, health check).
  • Your records arrive from your previous GP (this can take a little time).

Need help?

  • No photo ID or proof of address is required to register.
  • You can register even if you’re not “ordinarily resident” or don’t have a fixed address.
  • Tell us about any accessibility or communication needs.
  • If you’re stuck, call Reception and we’ll guide you through it.

Privacy & Security

Online registrations use the secure NHS service. If you send other forms online, your data is transmitted over the internet. We take care to keep information secure, but absolute security cannot be guaranteed.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 6pm.

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.

Temporary Registration

How to get care if you’re away from home or not registered with a GP.

Immediate help (up to 14 days)

If you become unwell while away from home, a local GP practice can treat you for urgent problems for up to 14 days. This is called immediately necessary care.

If you need care for longer than 14 days, please register as a temporary patient (see below).

Temporary patient (24 hours to 3 months)

You can register as a temporary patient if you are staying in our area for more than 24 hours and less than 3 months. You will stay registered with your usual GP at home.

If you live in our area long-term, please register as a permanent patient rather than using temporary registration.

Good to know

  • You do not need photo ID or proof of address to register, but it can help us find your records.
  • Practices must provide urgent care to anyone. For non-urgent care, a practice may not be able to accept you if they’re full or you live outside the area.
  • If you’re unsure what you need, call Reception and we’ll advise you.
