The Summary Care Record (SCR) is an electronic record that contains key information from your GP record — such as your medications, allergies, and adverse reactions to medicines. It helps health and care staff provide you with safer, faster care when you need it.


Why it’s important


Having a Summary Care Record allows authorised NHS staff outside your GP surgery — for example in hospitals, urgent care or pharmacies — to see essential details about your health when they’re treating you. This can be vital in emergencies or when your GP practice is closed.


What information it contains



    • Medicines you are currently taking

    • Allergies you suffer from

    • Any adverse reactions to medicines

    • (Optional) Additional information such as diagnoses, care plans, or long-term conditions — if you agree to share them


Who can access it?


Only authorised health and care professionals directly involved in your care can access your SCR. Every access is recorded, monitored, and audited to ensure security and privacy.


Do I have a Summary Care Record?


Most patients in England now have an SCR created automatically, unless they have chosen to opt out. If you’re not sure, you can ask at reception or check your record in the NHS App.


Can I opt out?


You do not have to have a Summary Care Record if you prefer not to. To opt out, please complete the official Summary Care Record Opt-Out Form and return it to the practice.
