Appointments With a GP – Nurse - HCA

You can make an appointment to see the doctor of your choice by telephoning the surgery, but choice cannot be guaranteed if your appointment is urgent. Reception will need to ask you some questions in order to direct you to the appropriate clinician, this is called care navigation, please do not be offended.

You can book in advance to see a Doctor of your choice up to a maximum of 3 weeks time.

Appointments can also be booked via the internet, using Patient Access on the patient.co.uk website. Please ask at reception if you wish to be registered for this facility.

Please arrive promptly for your appointment and check in via reception or our ‘Electronic Check in Service’ that will automatically let the receptionist and relevant clinician know you have arrived. You will be called for your appointment by an intercom system.

The practice has a policy to be proactive in encouraging attendance or cancellation of all appointments. If you are a Non-attendance it will be entered in your records and a letter will notify you each time.  If you fail to attend on 3 occasions within a 6-month rolling period you will be removed from the practice list. This also applies to children’s appointments where the whole family will be removed, as the parent/legal guardian is the responsible attendee on behalf of the child.

If you think you may have a urinary tract infection you are advised to bring a sample to reception and one of the nursing team will test the sample and inform you should you require treatment. An appointment is not required unless the patient is under the age of 16 years.

If you require a chaperone please ask when you make an appointment, although the doctor will offer one if he/she feels it necessary

Telephone Contact With the Doctor or Nursing Team

You may leave a message, which will be passed on to them for their attention after morning surgery or before evening surgery for a response and a member of staff may ring you back. Please ensure you leave the receptionist with the correct telephone number to contact you on.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

Home Visits

Home visits are for housebound patients or at the discretion of the doctors and are for seriously ill patients (who cannot attend surgery).  All requests should be made before 10.30 am.

Lack of transportation to the health centre is not a valid reason for a home visit.

You can be visited at home by a community nurse if you are referred by your GP. You may also be visited at home by a health visitor or midwife if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
